We ask that the form is fully completed to help us fulfill your request in a timely manner.
A response will be provided as soon as possible, and in any event within 1 month. Where we decline an application we will provide a reason. In some circumstances some parts of your record may be withheld if we feel that it is confidential or puts someone at risk.
